In consideration of the services rendered by Katy The K9 Keeper to my dog(s), I acknowledge reading, understanding, and accepting the statements herein. Please acknowledge the following policy statements by typing your initials in the boxes provided. If you do not agree to all statements, please do not complete this form, and contact Katy The K9 Keeper directly for more information.

Liability:

I understand certain “activities” that my dog may participate in (EX: including walks, playgroups, boarding & one-on-one play) at Katy The K9 Keeper could involve risk and possible injury. I further understand that not each and every potential risk can be listed but, nonetheless agree that the benefits associated with dog care outweigh the possible risks, therefore, I hereby voluntarily release and agree to hold harmless and indemnify Katy The K9 Keeper and its agents from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my dog’s participation in activities (care) at Katy The K9 Keeper.

Authorization of Medical Care:

If my dog is ill or injured while participating in activities during it's stay at Katy The K9 Keeper, KTK9K will make every reasonable effort to reach me pursuant to the contact information I have provided on this form and the enrollment form. However, if Katy The K9 Keeper is unable to reach me or my emergency contact, I consent KTK9K seeking appropriate veterinary care and I accept responsibility for any and all associated expenses. Katy The K9 Keeper will not pay any portion of veterinary expenses associated with seeking medical care for my dog if so necessary. Also, Katy The K9 Keeper will not be liable for treatment decisions made based on the vet's recommendations.

Vicious Tendencies:

I affirm that I am not aware of any vicious tendencies by my dog and have had no incidences requiring medical or legal intervention with another dog or human.

Agreement to Pay:

Katy The K9 Keeper accepts check, cash, credit cards (w/ 3% service charge), PayPal (Friends & Family), and Venmo. I agree to pay the service rates in effect for my dog’s care at Katy The K9 Keeper. I further agree to pay for any additional services requested such as bath, drop-off, and/or pick-up service. All services must be paid in full before a dog can head home.

Damage:

I accept the responsibility of paying for any damage to property and/or equipment caused by my dog(s).

Meet & Greet Prior to Stay:

Every dog must be evaluated by Katy The K9 Keeper prior to their stay. This is done by a scheduled meet and greet with either Katy or Charles. This evaluation is to assess the dog’s temperament and interactions with other dogs, as well as compatibility in the home atmosphere.

Right to Decline:

I understand that Katy The K9 Keeper reserves the exclusive right to decline or terminate care to any dog at any time for any reason.
